Freedom Hospice offers palliative care and support services to terminally ill patients and their families. We provide quality, comprehensive hospice care while remaining focused on meeting the physical, emotional and spiritual needs of patients and their loved ones.
Freedom Hospice's interdisciplinary team of professionals works closely with the patient's primary caregiver(s) and their attending physicians. The team might include:
- Hospice Medical Directors that oversee the selected hospice agency and supervises the patient's medical needs. Our on-staff medical doctor works with the patient's physician to control pain and manage symptoms, and leads the hospice team.
- Nurses that make regular visits and address the medical concerns of the patient. They explain the progression of the disease, teach family members how to perform physical care and coordinate medications. A certified nurses' aide may also assist with personal care needs such as bathing, changing bed linens and perhaps light housekeeping.
- Social Workers help patients and family members cope with life-limiting illnesses and ascertain their impact on the family. They foster open communication and find ways to fulfill individual and family needs while providing guidance about loss and grief.
- Pastoral care is important as hospice responds to the whole person. When patients have strong ties to their own church or synagogue, the hospice chaplain can arrange for service in the home. Hospice is interdenominational and has access to clergy of all faiths to provide pastoral care.
- Volunteers play an extremely important role, especially for families with little support. They provide assistance and support, without compensation, for the patient, families and your hospice team.
- Bereavement Coordinator collaborates with interdisciplinary team to promote coordination of bereavement for family members; bereavement support provided for 13 months after the death of your loved one.
Because hospice allows patients and families to direct their own care, at Freedom Hospice, every family is afforded a tailored plan of care. With expert counseling from our hospice team, patients and families can make sound decisions based on what is important to them. Most notably, hospice team members aim to provide an atmosphere of understanding, comfort and acceptance.
